ACCIDENT WAIVER AND RELEASE OF LIABILITY – THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS – READ CAREFULLY BEFORE SIGNING:

By signing below, I voluntarily agree to participate in a tour of the Salk Institute for Biological Studies ("Salk"). I understand that participation involves inherent risks, including injury, illness, or property damage, and I accept full responsibility for my safety and belongings. In consideration for being allowed to participate, I, on behalf of myself, my heirs, and assigns ("Releasors"), waive and release Salk, its directors, officers, employees, volunteers, and agents ("Releasees") from all claims or liabilities arising out of my participation, including claims I may not yet know about.

California Civil Code §1542: "A general release does not extend to claims that the creditor does not know or suspect to exist in their favor at the time of executing the release, which, if known, would have materially affected their decision. - I expressly waive all rights under §1542. Health & Safety: I certify that I am physically able to participate and accept responsibility for my own safety. I consent to receive necessary medical treatment in an emergency. Animals: Only legally required service animals are permitted on tours.

Image Release (optional): I grant Salk permission to use my name, likeness, and voice in photos, videos, or recordings for promotional purposes without compensation.

Acknowledgment: I have read and understood this waiver. I sign it voluntarily and intend it to be a complete and unconditional release to the maximum extent permitted by law. Please note: All members of your party over the age of 18 must individually click to sign as a condition of their participation in a tour. A parent or guardian must click to sign on behalf of any tour participant who is under 18.
